Our friendly staff may ask for your name and address while making your appointment. This is to ensure we have the correct patient.
Standard Appointments are 10 to 15mins. If you require a
longer appointment please let our staff know when you call.
On arrival please check in with our reception staff and notify them of any changes to your contact details.
Medicals
Corporate medical assessments usually involve paperwork. If you already have this completed please bring it with you. If you do not have the paperwork we would please request you arrive 10 to 15 min before your appointment time to complete the relevant forms.
The forms for recreational and commercial dive medicals are available on this website (click here). A recent chest x ray is required for a commercial dive medical. Please bring this with you or call reception to arrange prior to your consultation. |
